Fernandez Community Center is a Certified Care Management Agency by the North Carolina Department of Health and Human Services.
Tailored Care Management (TCM) coordinates all aspects of a person’s healthcare needs to improve their well-being and quality of life. Coordination across a member’s whole-person needs, including physical health, behavioral health, 1/DD, traumatic brain injuries (TBI), pharmacy, unmet health-related resource needs, and other social determinants of health. TCM involves a care management comprehensive assessment to identify the member’s needs and the development of a Care Plan to address those needs.

- Specialized support for people living with mental illness, substance use disorders, intellectual and developmental disabilities, and traumatic brain injuries.
- Contact with one Care Manager who will listen to your needs and coordinate care among your team members.
- Support for additional needs such as housing, employment, food insecurity, physical health, finances, and safety which may affect overall health and well-being.
